Regular, full-time employees are eligible to participate on the first day of the month following 90 days of continuous employment. Medical coverage includes prescription coverage and is available for employee only or family. Employee contributions are made through payroll deduction.

Dental Insurance - Delta Dental (Voluntary)

- Orthodontics (children up to the age of 19) $1,000 maximum lifetime coverage

Vision Insurance - Anthem Vision (Voluntary)
- No annual deductible
- $20 in-network eye exam copay
- $25 in-network copay for lenses
- Frames covered at $120 allowance, then 15% off remaining balance (every 24 months)

Short-Term Disability - Lincoln Financial Group (Voluntary)

- 60% of base pay, up to maximum monthly benefit of $5,000

- Additional life insurance is available in $15,000 units up to a maximum of $60,000

- Responsible for paying 100% of the cost of your coverage which will be made through payroll deduction on a post-tax basis. Benefit amounts reduce at age 70
